ability to request available Merck product samples, vouchers, and/or coupons for use with
their appropriate patients.
Only licensed US health care professionals may register for Merck Sample Center.

required information.
All fields that include a red asterisk (*) are required. All of the information
you submit on this form is protected under the SymmetryRx™ Privacy Notice.
Once you've logged in to Merck Sample Center, click on the "My Profile" link to
edit your account information. To change your password, click the "Change" link
under "Password" on the profile page.

Your samples, vouchers, and/or coupons are shipped to your office as soon as your
request is validated and processed.
Validated MDs, DOs, and other licensed practitioners (depending on state-level requirements)
can expect to receive samples, vouchers, and/or coupons within 14 business days
of Merck receiving the signed Sample Request Form.
